In an effort to give this important work the widest distribution, we have reformatted the file as an htm file. It's still a large file but no where near large as the original pdf file.

We've tried to make this version as clean as possible but you may notice odd spaces between letters in the middle of a word or some extra or odd punctuation, that we may have missed. While this may be distracting, it does not take away from the contents.

Some of the Russian word while looking as if they need correction are, in fact, correct.
